Applying made simple:

Applying for services is easy and requires either a direct refferal from your Regional Center or self-referral.  The process includes a phone intake where we determine if our program can meet the clients' needs.  Second, if the client meets our initial intake criteria, we will schedule an in-person assessment and work with the client to determine the type of services we can potentially provide.  In some cases, we may need to decline the client, but may offer recommendations for skills that can be aquired to make the client more appropriate for our program.  ALL referrals will need to have SLS in the client IPP.  We do not provide direct services without Regional Center approval.

If you are interested in SLS services, referrals can be made directly or through your local Regional Center Service Coordinator.  All referrals will need to be validated for funding. 

 Due to the specific nature of our program type, not all individuals will be found eligible or appropriate.  Our primary goal is to insure that each prospective resident is a good fit for our program and that includes a thorough assessment process and trial period whereas, individuals will be assessed in multiple areas of functional and emotional skills to help evaluate if we have the program components available to meet the needs of your loved one.  We are a specialized program for individuals who cannot live successfully in a lower level of care for a variety of reasons. Our goal is client success!

Applicants will be evaluated on the appropriateness of our program based on the following criteria:
              The individual:
1.      Has a developmental  disability and is a Regional Center recipient;
2.      Resides in El Dorado, or adjacent county;    (EDC residents will receive priority over non-EDC residents)
3.      Can achieve individual service goals;
4.      Is between the ages of 18-64 years old;
5.      Can have personal needs met with 1:1 support & shadowing;
6.      Is medically stable;
7.      Has cognitive ability to participate in the program;
8.     Lives independently or will live independently at the start of SLS.
